Under limited supervision, the Population Health Program Manager develops, implements, and manages a project management infrastructure for effectively managing the reporting of all care redesign programs assigned to the Population Health Team. The project management plan includes establishing, tracking, and reporting on major milestones and performance outcomes associated with various population health programs. The Program Manager will also support the health system's Community Benefit work. The position will require close collaboration with the entire population health team and other members of the health system.

Under limited supervision, the Population Health Program Manager develops, implements, and manages successful participation in assigned Value Based Care and Population Health efforts. Effectively manages, and communicates program related information, including performance, requirements, and reporting. Develops and or advances care redesign and clinical transformation initiatives within GBMC Health Partners and GBMC the Acute Care Hospital. The position will require close collaboration with other population health team members and with other departments within the health system.

Education

Bachelor's Degree and Master's Degree required

Experience

  • 3-5 years directly related and progressively responsible experience in project management and data analytics
  • Experience working in a team-oriented, collaborative environment
  • Understanding of federal, state, and private-payer value-based contracts
  • Expertise in relationship management and team building

Knowledge, Skills and Abilities

  • Proven project management and program management skills, including work plans and other tools.
  • Analytic ability to problem solve and develop goals, objectives, policies and procedures
  • Ability to synthesize information related to project milestones, interim deadlines, content and context of deliverables, responsible individuals, and critical resources
  • Ability to coordinate effective solutions to organizational and programmatic needs
  • Ability to assign responsibility for deliverables and track completion across a team consisting of members from multiple departments
  • Ability to build and maintain relationships across departments and levels of seniority
  • Proficiency in oral and written communication to a variety of audiences including staff, physicians, and executive leadership
  • Proficiency in using Microsoft suite software, including SharePoint and navigating between multiple IT systems

Licensures, Certifications

Project Management Professional (PMP) or other project management certifications preferred

Physical Requirements

  • Normal office environment. Ability to sit for 60-90 minutes at a time, to lift up to ten pounds maximum and occasionally lifting and/or carrying small items

Principal Duties and Responsibilities

  • Responsible for the development, implementation, and management of an integrated population health project management plan capturing data and information for specific care redesign programs assigned to the population health team
  • Establish, track, and report on major milestones and performance outcomes associated with federal, state, and commercial payer population health programs. Prepare and present dashboard-level information on program information to management
  • Responsible for managing the submission of required program reports for all assigned programs. Ensure all required submissions are submitted on-time, are accurate and are stored appropriately for record retention purposes
  • Take a central role in team meetings and a supporting role in system-wide leadership forums as an expert in the schedule of deliverables and milestones for the population health programs/initiatives that GBMC participates in
  • Assist in the tracking of budgetary and financial reporting requirements for the population health department, including high level milestones for collaborative or contractual arrangements with other health systems or organizations
  • Other duties than as assigned, towards benefiting Patients and GBMC Health Partners

Pay Range

$72,782.10 - $123,729.56

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
