This Social Work Patient Navigator will be responsible for:

  • Providing education and emotional support to patients and families and helps guide them through the healthcare system.
  • Serving as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing supportive care services within BCH and in the community.
  • Meets with/calls patients and families to review the implementation of care plans.
  • Assessing patients for referrals to resources.
  • Assisting patients and families with scheduling multidisciplinary appointments as needed.
  • Assisting patients with applying for insurance.
  • Maintaining relationships for accessing community-based resources. Providing outreach and maintaining positive working relationships with providers/administrative staff (including physicians, office staff, nurses, and social services staff).
  • Documentation of patient care and required data.
  • Attending and participating in care planning meetings to prepare patients for discharge or continuing care.

To qualify, you must have:

  • High School Diploma/GED required. Bachelor's Degree in Human Services, Social Work or related field highly preferred.
  • One year of previous applicable professional experience is required.
  • The ability to work with a diverse, multi-cultural population and dedication to working with families struggling with financial hardship and medical complexity.
  • Analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.
  • Ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
  • Diverse candidates encouraged to apply. Fluency in Spanish is preferred.

Please note: During a public health emergency, individuals in this role may be expected to take on additional duties to respond to organizational needs.
