WhyM Physicians? Our Adult Congenital and Cardiovascular Heart Disease Clinic is one of the few in the country serving adults born with congenital heart conditions and inherited heart disease. The ACHD Team is looking to add another nurse team member as our population grows who values team collaboration and a passion for caring for patients born with congenital heart diseases. This role integrates clinical expertise in congenital cardiovascular nursing with case management, patient education, and care coordination across the continuum-from outpatient management to inpatient hospitalization and procedural planning. The role is responsible for ensuring patients receive high-quality, evidence-based, and patient-centered care by coordinating services, facilitating communication between providers, and guiding patients through complex healthcare systems.
