JOB PURPOSE: The Outreach & Intake Coordinator is responsible for building relationships with community partners, engaging underserved and vulnerable populations, and connecting individuals to shelter, housing, and supportive services. This position conducts intake screenings and coordinated early recovery and housing assessments to determine program eligibility and facilitate access to appropriate shelter and resources. Working within an early recovery center environment (day or night), the coordinator promotes awareness of shelter and housing programs, organizes outreach activities, and ensures effective communication between the organization, clients, and community stakeholders. The role requires high energy, professionalism, strong presentation and motivational skills, and the ability to maintain composure in a high-volume, fast-paced setting while serving individuals with empathy and respect. ESSENTIAL JOB RESPONSIBILITIES: * Conduct initial intake screenings and coordinated early recovery and housing needs assessments to determine program eligibility using a trauma-informed, strength-based approach. * Screen clients for program eligibility and provide orientation to available services. * Provide general information and appropriate community referrals to individuals who are ineligible or unable to be scheduled with a Service and Care Coordinator due to program capacity. * Assist eligible participants with intake and assessment of individual needs, create initial service plans, and set up client files for assignment to Case Managers. * Develop and maintain a comprehensive referral network with community partners to connect clients with essential services, including mainstream benefits and community resources. * Establish and maintain effective, trust-based relationships with participants, always modeling positive interactions and communication.Track and maintain an up-to-date list of residents requiring new or updated coordinated assessments and ensure accurate data entry and documentation in HMIS. * Track outreach and intake activities, prepare reports, and evaluate program effectiveness. * Develop and maintain strong working relationships with landlords, property managers, and housing providers to support participants in obtaining and maintaining stable housing. * Conduct community outreach and networking activities, including presentations at events, active participation in community meetings, and promotion of the organization's housing and support programs. * Maintain regular communication with a multidisciplinary team across programs to coordinate care and promote housing stability for participants. * Document all participant interactions through detailed case notes, submit incident reports as needed, and ensure timely and accurate data entry into HMIS. * Support the organization's mission to "Empower people to build better lives for themselves, their families, and their communities" by consistently demonstrating exceptional customer service in all interactions.Perform other duties as assigned. * Demonstrate Exceptional customer service, in Everything you do, by placing the child, family, Veteran or client first to support our mission to "Empower people to build better lives for themselves, their families, and their communities." ESSENTIAL QUALIFICATIONS: EDUCATION: High School Diploma or GED required; Bachelor's Degree in Social Work, Sociology, Psychology, or related field, preferred. EXPERIENCE: 2+ years in a customer service-focused environment. Experience with homeless, low-income, veterans & their families is a strong plus. ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion. Position is in person at the program center. LICENSES: Driver's License with clear record. VEHICLE: Must have daily use of a vehicle without prior notice. OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
