Summary

The Intake Specialist RN in Home Health coordinates the clinical and administrative functions of Intake and oversees staff support to the Intake Team Leader and Manager.



Responsibilities

  • Receives telephone and faxed referrals from referral sources including but not limited to Physicians, Hospital Social Workers, Hospital Discharge Planners, Nurses, patients and family.
  • Obtains demographic, clinical and payer information from client's physician's office and other referral sources to complete referrals.
  • Completes telephonic data entry of referrals into database.
  • Effectively communicates patient/referral information to appropriate Intake Staff and Branches.
  • Effectively documents all communication related to referrals.
  • Receives orders from Physicians and communicates to branch office promptly.
  • Communicates outcome of referrals with referral sources and physicians.
  • Coordinates communication of referrals and staffing cases with Branches.
  • Utilizes clinical and administrative knowledge to determine acceptance of appropriate referrals.
  • Disseminates referral paperwork to assigned Case Coordinator to review form completion and prepare for pre-billing.
  • Triage incoming calls and messages from referral sources, etc. to Intake Team Members. Relays messages as appropriate or transfers to voice mailbox. Specialist must exercise discrimination and diplomacy on obtaining information from callers for persons not immediately available.
  • Provides general information about agency services to patients, family, and other referral sources.
  • Completes special tasks, projects and audits as assigned.
  • Consults with Intake Team Leader or Manager regarding negotiating discounts with Case Managers and non- contracted payer sources.
  • Completes review of Intake Coordinator referrals.
  • Serves as acting Intake Team Leader in Intake Team Leader's absence.
  • Serves as a resource for Intake Coordinators.
  • Assists Intake Team Leader with overseeing the clinical and operational functions of the team.
  • Assists Director with orientation of branch staff in Intake.
  • Participates in on-call rotation as necessary.
  • Acts as Charge Nurse on Saturdays.
  • Reviews On-Call Nurse activities.
  • Informs Marketing and Sales Leader of potential contract possibilities.
  • Attends and participates in workshops, continuing educational programs, seminars, etc. to keep abreast of current departmental changes.
  • Attends and participates in annual in-service training programs.
  • All other duties as assigned.



Education and Experience

  • Must possess current licensure as a Registered Nurse in the state of practice.
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.
  • Current CPR certification required.
