Key Position Details:

Position requires candidate to float to different skilled nursing facilities within their assigned region

  • 1.0 FTE (80-hours per two-week pay period)
  • 8-hour day shifts
  • No weekends

Job Description:

Serves as the direct liaison between Allina Health and assigned facilities to support the implementation and growth of the Preferred Provider Network. Responsible for implementing strategic programs and performance improvement initiatives across assigned sites. Coordinates patient placement to support hospital throughput and readmission initiatives.

Principle Responsibilities

  • Implements current and new programs and services.
    • Manages relationships with internal and external partners to build trust and drive engagement in strategic and operational initiatives.
    • Assists with implementation of identified programs and services at assigned sites.
    • Collaborates with site leadership and stakeholders to ensure successful adoption of identified programs.
  • Implements performance improvement initiatives.
    • Supports the implementation of multiple performance improvement initiatives.
    • Analyzes data to identify performance improvement opportunities and guide strategic design solutions.
    • Prepares and presents actionable deliverables to internal and external stakeholders.
    • Partners with external stakeholders to implement targeted action plans and monitor progress.
    • Ensures continuity and system integrity of services post-discharge from SNF through coordinated stakeholder engagement.
  • Supports Allina Hospital throughput and readmission initiatives.
    • Supports placement of patients through escalation process.
    • Supports rollout of initiatives at sites that support readmission improvement goals.
    • Proactively identifies sites not meeting census or clinical goals through site observations, relationship building and available data.
  • Other duties as assigned.

Required Qualifications

  • Bachelor's Degree in Healthcare
  • 2+ years clinical experience in skilled nursing facility

Licenses/Certifications

  • Registered Nurse licensure preferred-Active in the State of MN
  • Clinical license in area of expertise preferred-Active in the State of MN

Physical Demands

Pay Range

Pay Range: $37.67 to $51.63 per hour The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
