The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Community Health Worker will identify, coordinate and link HIV positive clients to medical care from all access points. Provides individualized support for clients to overcome barriers to care, improving treatment outcomes. Identifies and supports client concrete needs such as healthcare benefits, entitlements, housing, etc. Delivers health education sessions to increase health literacy and empowerment. Serves as primary contact and liaison for clients and care team to ensure optimal treatment adherence and retention in care.

Qualification

Required:

  • Community Health worker will have a bachelor's or equivalent level of education preferred, training in a related social service or human service field, and experience working in the community and with patients with co-morbid conditions.

Certifications and Licenses Required (if applicable):

  • Formal Community Health Worker Certification a definite plus.

Essential Functions:

Specifically, the Community Health Worker is responsible for:

  • The CHW will identify, coordinate and link HIV positive clients to medical care from all access points.
  • Provides individualized support for clients to overcome barriers to care.
  • Identifies and supports client concrete needs such as healthcare benefits, entitlements, housing, etc.
  • Manages caseload of identified vulnerable populations to ensure compliance with medical care and improved health outcomes.
  • Serves as primary contact and liaison for clients and care team to ensure optimal treatment adherence and retention in care.
  • Reconciles patient out of pocket costs billing to be paid by grant funds as eligible
  • Facilitates insurance verification for all scheduled FTC patients prior to appointments and manages associated follow up tasks that ensure successful visits, reduce no shows
  • Actively participates in the CQI Project Team for FTC at Rutgers
  • For Ryan White EHE grant funds, prepares required monthly reporting documents and supports all audit and site visit activity
  • Implements, monitors and analyzes all aspects of our Take Control patient cohort to bring FTC patient viral load suppression rate in alignment with national expectations of 90 percent.
  • Facilitates team clinical huddles to case conference, prepare for medical service, share safety topics, publicize good catches and increase appointment retention
  • Manages and updates recertification report in data base for clients who receive direct services within the last 6 months or 12 months
  • Facilitates the process of providing short-term rental assistance to eligible Ryan White patients including recruitment, completion of applications, requesting payment and monitoring available funds.
  • Candidate must have the ability to demonstrate the delivery of culturally sensitive services to reduce stigma, ability to provide trauma-informed care and be able to function effectively within a team-based, inter-professional approach.
