Description

Nexus Family Healing is looking to hire a Admissions and Transitions Coordinator to join our newest youth crisis and stabilization center set to open in fall 2025 in Hennepin County!

This new youth crisis and stabilization center is a 13-bed, short-term residential service is for Hennepin County youth with complex mental and behavioral health issues looking for appropriate care. The service will help to reduce the number of youth who are stuck in inappropriate care settings such as emergency rooms, police stations, juvenile detention or unstable home situations and provide treatment and therapeutic supports while triage, assessment, and transition planning take place with families.

Pay and Schedule:

  • Full-time Hybrid opportunity. Working in office and from the community.
  • Location: 1800 Chicago Avenue in Minneapolis
  • Pay Range: $21-23 hourly

Position Summary: Our Admissions and Transitions Coordinator coordinates ongoing therapeutic services and community support necessary to ensure clients can receive stabilization care.

Primary responsibilities:

Client Consultation & Treatment Planning

  • Assists clients in linking to services, identifying barriers, and connecting with supports prior to discharge.
  • Provides consultation and support to families, youth, and adults in need of services.
  • Work closely with the teams to create stabilization plans, discharge plans, and supported engagement.
  • Develops treatment plans and utilize a variety of treatment approaches with clients.
  • Provides assistance to families regarding resources available to the families and youth that will support the healthy functioning of the family unit.; serves as a resource to families for consultation and crisis intervention. f. Screens direct referrals for needs, assist in triaging individuals.
  • Completes intake screening and assessments as necessary.

Service Delivery

  • Works effectively with outside organizations to successfully implement plans for adults and children entering crisis services.
  • Works to bridge relationships with adults, families, youth, and outside organizations and provide role modeling for advocating for needs.
  • Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients.
  • Create aftercare plans and follow up.
  • Maintains and completes paperwork and compiles documentation required by the organization and for the governing/accrediting agencies; maintain current files on all individuals enrolled in services.
  • Produces informal and formal reports as requested by outside agencies, parents, referral agencies, courts, etc.
  • Facilitates discharge planning meetings and develop a discharge plan that meets the individual's needs.
  • Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients.

Outside Organization Collaboration & Support

  • Communicates with families, legal systems, schools, mental health providers, probation, and social service agencies; works effectively with outside organizations to successfully implement plans for adults and children.
  • Work to bridge relationships with adults, families, youth, and outside organizations and provide role modeling for advocating for needs.
  • Maintains database of community resources and contacts for outside providers and organizations; collects data on services, linkages and additional outcomes as identified.
  • Completes the aftercare process of discharge planning, follow up on connections to outside services and conduct satisfaction services for providers and individuals receiving services.

Policy, Procedure, Compliance & Training

  • Ensures confidentiality of all treatment and services information pertaining to all individuals and their families
  • Participates in staff and supervision meetings and required in-service meetings.
  • Exhibits clear, professional, respectful, and effective communication when interfacing with clients, coworkers, families, and external contacts and willingly share knowledge, information, and resources appropriately.
  • Demonstrates concern and empathy towards others as needed, while maintaining appropriate boundaries
  • Demonstrate self-awareness and willingness to process one's own emotional and behavioral responses to situations in the workplace and demonstrate professional use of self in day-to-day work situations.
  • Interacts with and appreciate people from diverse cultural, social, economic, and religious backgrounds.
  • Demonstrate professionalism and work accountability.
  • Maintain a schedule that is adjustable and flexible to meet changing work needs and demands of clients and their families.
Requirements

Required Education and Licensure:

  • Bachelor's degree and/or
  • 5+ years of experience in social work, case management or similar duties as outlined.
  • Required to meet MN Mental Health Practitioner definition as defined in the MN statutes/section 245.462, subdivision 17 located at https://www.revisor.mn.gov/statutes/cite/245.462
  • Valid driver's license required. Must meet state regulating agency and Nexus Home Office driving requirements.

Preferred Education and Experience:

  • Bachelor's Degree in Human Services
  • 2+ years of experience managing a caseload related to youth/families.
