Licensed Practical Nurse, Home Health

As an LPN for the Home Health team, the partner will perform nursing services that require the use of physical, social sciences and nursing skills in the care of the sick, the conservation of health and the prevention of disease in patients home. Responsible for the care of the patients assigned as directed by the registered nurse. Other duties as assigned. Responsibilities Essential Duties and Responsibilities
Education and Experience Graduate of an accredited LPN Program OR Wisconsin: Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing. Illinois: Have completed the designated curriculum of a Ladder Option Approved Designated RN Program. A minimum one year relevant experience preferred.
