POSITION SUMMARY: Responsible for collaborating with the multidisciplinary Office-Based Opioid Treatment (OBOT) Program Team in caring for opioid dependence and poly substance issues at BMC and through the State of MA. Cares for patients with substance use disorder in the primary care practice. Provides skilled nursing services to opioid dependent patients and their significant support systems. Ongoing follow-up, triage, teaching, inductions, stabilizations, and monitoring of patients in opioid treatment program receiving buprenorphine and naltrexone. Provide primary care of patients within the opioid treatment program, as well as support and services to patients from other settings referred to program for stabilization and assessment. Collaborates with team members, clinic settings at BMC and outside referral sites, works with providers from addiction settings in assisting patients and supporting their referral needs. Works under direction of OBAT Clinic RNs, and supervises the OBAT MA. Position: OBAT LPN Department: Research - General Internal Medicine Schedule: Full Time ESSENTIAL RESPONSIBILITIES / DUTIES: 1. Clinical Care/ Referral 1.1 Assists with providing patient care: records vital signs; discusses and documents chief health concerns. 1.2 Performs and record specialized testing included but not limited to EKGs, glucose and TB testing. 1.3 Prepares and safely administers medications, including but not limited to immunizations, flu shots, and other injections. 1.4 Performs nursing type duties specific to the department. For example, may apply dressings, perform ear irrigations and other wound care as directed. Specific duties vary by department and specific population. Performs duties as directed by supervisor, manager and/or physician. 1.5 Coordinates patient prescriptions. Makes phone calls to pharmacies. 1.6 Acts as a liaison between nursing agencies, VNA and other health agencies. 1.7 Uses computer to schedule appointments necessary for follow up appointments or other ancillary appointments, as appropriate. 1.8 Performs cross-booking appointment scheduling as appropriate. 1.9 Responds to telephone and EMR requests. 1.10 Acts as a liaison to facilitate coordination of patient referrals and forms to other specialties which may require utilizing knowledge of eReferral and/or Clinical Information Exchange (CIE). 1.11 Performs substance abuse screener utilizing electronic screening tool, inputs this and forwards for review and plan to nurse manager 1.12 Meets with walk ins and assists them in accessing referrals to treatment, places on wait list and obtains screener when appropriate. 1.13 Follow up visits with patients under the direction of the OBOT RN's 1.14 Deals discreetly with sensitive information, maintaining confidentiality at all times. 1.15 Obtains and enters patient information and procedure data into hospital computer systems. 1.16 Responds to patient phone inquiries regarding office appointments, dates for specialty appointments, and other referred services such as radiology, surgery, etc. 1.17 Monitors patient flow and patient satisfaction. Prospectively identify potential issues. Trouble-shoot issues and work with Supervisor, and other managers to ensure efficiency. 1.18 Coordinates the patient flow and schedules for the supported Physicians and other Providers to ensure accurate and efficient daily operation. Training/Orientation Support Attends scheduled training sessions for systems upgrades or newly acquired clinical systems and all trainings as required. Other Support Duties
Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and patient care can be provided. Utilizes hospital's RESPECT standards as the basis for decision-making and to facilitate the division's hospital mission. Follows established hospital infection control and safety procedures OTHER DUTIES: Perform other duties as needed. JOB REQUIREMENTS EDUCATION: Graduate of an approved school of practical nursing. CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED Current license in the Commonwealth of Massachusetts to practice as a Licensed Practical Nurse. CPR & BLS certified upon hire (or within first 90 days of employment). EXPERIENCE: Requires a minimum of 2 years current experience in Ambulatory Office Setting. Experience in Adult Primary Care Medicine highly preferred. KNOWLEDGE AND SKILLS:
