Description This position provides health coaching, under the Basecamp Prevention + Wellness suite of programmatic offerings, with an emphasis on behavioral change modification in the areas of movement, nutrition and wellness. The Health Coach functions independently within community partner settings, onsite programmatic settings and within care clinics, while under direction of the program medical director and clinical programs director. The health coach develops systems and structures that work in concert with our prevention and wellness programming. The Health Coach is responsible for developing relationships with participants/patients through a combination of communication and methods aimed at increasing awareness, enhancing motivation, changing behavior and creating an environment that achieves optimal health and quality of life. Health Coaches promote Basecamp Prevention + Wellness tools and services to support participants health that align with wellness industry best practices. Coaches work collaboratively with participants/patients and stakeholders to develop and coordinate services that promote sustainable lifestyle change, such as movement, nutrition, wellbeing, nicotine cessation, for example. This position is responsible for researching, developing, implementing, tracking and evaluating health improvement programs and outcomes in alignment with Providence Heart Institute's Basecamp Prevention + Wellness strategies around lifestyle behavioral change for community health.
