The Nursing Support Coordinator supports the nursing team and helps with day-to-day operations while adhering to the organization's objectives, working under the direction of the Director of Nursing.

Responsibilities
- 1. Coordinates patient admissions to the Grimes Center in conjunction with hospital case managers and the healthcare team, making sure that correct information is recorded in patient records.
- 2. Serves as a representative of nursing and collaborates in the development of interprofessional learning initiatives and activities.
- 3. Protects health information confidentiality through communication with appropriate licensed health professionals and obtains HIPPA authorization to release such information.
- 4. Partners with management to provide staff with guidance and education on departmental procedures and opportunities for development, reviewing standards of clinical care, workflows, and documentation in EPIC.
- 5. Coordinates and gathers information from multidisciplinary team to obtain authorizations and other insurance updates.
- 6. Continually monitors the effectiveness of support provided and makes necessary changes and adjustments as needed.
- 7. Acts as a liaison in establishing and managing service agreements with providers, ensuring all services are delivered as per the plan.
- 8. Understands and recognizes the importance of maintaining a hygienic, safe environment and physical surroundings.
- 9. Performs other duties as assigned.
Qualifications
EDUCATION High school diploma or equivalent required. EXPERIENCE A minimum of one (1) year of experience in rehabilitative, geriatric nursing, or healthcare setting required. LICENSURE Current CT Registered Nurse (RN) license required. SPECIAL SKILLS Excellent organizational, multi-tasking, problem-solving, interpersonal, and time management skills with a strong attention to detail Ability to work effectively within a healthcare team Demonstrates ability to maintain standards of care rendered at Grimes Center per the State of Connecticut Public Health Code, the governing body of the facility, and the State and Federal agencies. Familiarity with the admission and discharge process.
