Qualifications:

Required:

• Graduate from an accredited School of Nursing.
• Bachelor's Degree Required.
• Minimum (2) two years of acute care nursing experience.
• Knowledge of Case Management roles, functions, and activities.
• Strong communication and organizational skills.
• Proficient computer skills.
• Strong critical thinking and assessment skills.
• Knowledge of system applications and EMR.

Certifications and Licenses Required:

• Active NJ RN license or compact RN license with NJ endorsement.
• BLS from the American Heart Association.

Scheduling Requirements:

• Day Shift, 40 Hours Per Week

Essential Functions: Responsible for ensuring the appropriate placement of patients throughout the hospital. Manage the bed placement and associated operations within the Logistics and Bed Board Center, supports the patient logistics program, including oversight of turnaround times, coordination of logistical operations, and staff management, collaborates with hospital administration, nursing leadership, medical staff, physicians, environmental services and transportation, ensure timely and accurate patient throughput and care, Supervise the patient logistics team, including bed board and patient logistics nurses, Collect and analyze objective and subjective data for effective patient placement with recommendations for operational improvements, Tracks and trends barriers to patient flow, makes recommendations and develops action plans to improve processes and systems. Prepares analyses to identify opportunities for improvement in admission and discharge planning. Communicates with physicians and nurses regarding admissions based on bed availability, treatment plan and admission criteria. Initiates and maintains communication throughout the day with the Emergency Department, Inpatient, Procedural areas and other areas. Works with Case Management, Nursing, Environmental Services and other departments to proactively plan for patient admissions, discharges and transfers for effective patient flow. Circulates through the hospital evaluating all needs for patient placement. Ensure that the department is in compliance with all applicable policies, laws and regulations.
