Full Time I Second Shift (4x10's)

Hospital Pharmacy I Hamburg & Lexington, KY

Pharmacy Services of Baptist Health Lexington provides pharmaceutical services to patients across the continuum of care. These services include, but are not limited to verification of medication orders, drug distribution, sterile product preparation, hazardous drug preparation, and utilization of automated dispensing cabinets.

Description:

  • Interprets and reviews medication orders for correct drug, dosage, route, frequency, allergies, therapeutic duplication, interactions, contraindications, and potential adverse events for all ages served.
  • Ensure drugs are available to patients at the time they are needed to avoid delays in therapy and promote positive patient outcome.
  • Provides comprehensive decentralized or pharmacy clinical services including, but not limited to, anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, and nutritional support services.
  • Works with inter-professional patient care teams to help patients with transitions to care, including but not limited to helping prepare patients for discharge.

Requirements:

  • Graduate of accredited College of Pharmacy.
  • Kentucky Board of Pharmacy license in good standing as applicable to practice site.
  • 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
  • Basic Life Support (BLS) certification is required. If not already certified, must obtain BLS within 30 days of their hire date.
