Greater Lakes Mental Health is a key component of MultiCare's Behavioral Health Network, providing mental health and substance use disorder treatment for over 10,000 individuals annually across Pierce County. Providing services for adults, children, and families with Medicaid coverage, Greater Lakes offers 6 convenient mental health care locations including a 16-bed inpatient facility covering medically necessary voluntary and involuntary services to adults. Our experienced teams, veteran nurses, and robust training plans are designed to support your success and growth in facilitating recovery-oriented and trauma-informed treatment.

MultiCare Behavioral Health is seeking an experienced and compassionate Family Nurse Practitioner to join our thriving and well-supported behavioral health team. This role will primarily support the Greater Lakes Homeless Outreach Stabilization and Transition (HOST) program, delivering field-based care to adults (ages 18-59) facing serious behavioral health and substance use challenges.

Greater Lakes Homeless Outreach Stabilization and Transition (HOST) program provides outreach-based treatment services to individuals with serious behavioral health challenges including substance use disorder (SUD). This multidisciplinary teams can provide SUD, medical, rehabilitative, and peer services in the field to individuals who lack consistent access to these vital services.

Position Highlights:

  • FTE: 0.8 (32 hours/week)

  • Schedule: Monday-Friday, flexible daytime hours

  • Location: Greater Lakes Main Building, Lakewood, WA (with community-based outreach)

  • Shift Based Compensation per 1.0 FTE Years of Experience post residency: 0-3: $171,600 4-9: $179,200 10+: $183,000

Position Summary

Provides psychiatric services to members residing in the community enrolled in outreach services including, but not limited to, consultation, evaluation, medication management, and follow-up. Supports various clinical teams which could include PACT, ICSRP, ECS, outpatient psychiatry providers, clinicians, peer support specialists, collaborative care teams, primary care providers, allied support staff and others. Advises urgent care, emergency services, inpatient providers, primary care providers on appropriate medications, treatment strategies, educational support on diagnosing and managing behavioral health conditions.Responsibilities
*Provides psychiatric services to clients enrolled in community outreach programs, including medication administration and monitoring as well as general nursing care and patient education.
*Collaborates with internal and external medication prescribers and other clinical staff to ensure effective patient care.
*Services could be provided in the community via outreach.
*Responsible for remaining on call if in the event program staff need to consult regarding medical treatment that may be needed for patients.
*Follows community standards for prescribing.
*Maintains a flexible schedule during scheduled shift depending on patient needs.
*Provides case management support to patient as needed in coordination with team members.
*Provides and receives effective consultation, internally and externally, to coordinate patient care effectively.
*Monitors quantitative measures, blood pressure, heart rate, AIMS testing, laboratory work, weight, as clinically appropriate.
*Assists other community outreach staff with general client care management throughout the shift as needed, including participating in crisis intervention and management of patient emergencies.
*Demonstrate effective de-escalation skills.
Requirements
*Meets state of Washington licensure requirements by the time of employment.
*Advanced Reg Nurse Practitioner with prescriptive authority license - Washington Department of Health.
*American Nurses Credentialing Center Board Certification as a Psychiatric-Mental Health Nurse Practitioner (Across the Lifespan) Certification (PMHNP-BC) or if one of the following active certifications (available for renewal only).
*Adult Psychiatric-Mental Health Nurse Practitioner Certification (PMHNP-BC)
*Adult Psychiatric-Mental Health Clinical Nurse Specialist Certification (PMHCNS-BC)
*Child/Adolescent Psychiatric-Mental Health Clinical Nurse Specialist Certification (PMHCNS-BC)
*Current Health Care Provider level BLS certification such as American Heart Association or American Red Cross.
*DEA, NPI & prescriptive authority.

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $171,600.00 - $183,000.00 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant's years of experience align.
