Group Health Cooperative of South Central Wisconsin. BETTER TOGETHER The Certified Substance Abuse Counselor (CSAC) works collaboratively with a multidisciplinary team of addiction medicine physicians and advanced practice providers, registered nurses and other staff to coordinate, support and deliver evidence-based, high-quality substance use disorder treatment. The CSAC is responsible for delivery of evidence-based diagnostic assessment and treatment to adults with substance use disorders. The incumbent will receive day to day guidance, administrative supervision, and clinical consultation from the Associate Medical Director-Substance Use Disorder, with additional accountability to the Behavioral Health Services Manager. This position will provide both initial intake assessments and follow up psychotherapy care as a member of the substance use disorders program. The CSAC will collaborate with their patients to develop and address individualized treatment plans that include accurate diagnosis and specified treatment goals. Treatment benefits and progress toward goals will be monitored and reviewed regularly, utilizing measurement tools, and adjusting treatment focus as appropriate. This position may also coordinate care with primary care, other specialty providers, and the other systems in which their patients interact (e.g., families) as needed. The CSAC may provide crisis support to GHC-SCW members, including triage and risk assessments, completing safety planning, and determining acuity and appropriate level of care for members seeking substance use disorder treatment services. The CSAC will complete documentation in the electronic medical record in a timely manner per department and organization protocols, and chart in a manner that is patient-oriented and professional. The CSAC will work collaboratively with addiction medicine physicians and advanced practice providers and be an active participant in substance use disorder department meetings.
