About This Opportunity
AccordCare is seeking a Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. What You Will Do
As a vital member of our home health team, you will:
* Provide skilled nursing care tailored to each client's unique needs
* Monitor health conditions and document changes in clinical status
* Administer medications and perform wound care
* Deliver G-tube, trach, or vent support when required
* Educate and guide clients and families to support ongoing health at home
* Communicate effectively with the care team to ensure continuity of care
Who We Are Looking For
* Licensed as an LPN and in good standing with the Georgia Board of Nursing
* Experienced in a clinical setting, with at least one year preferred
* Certified in CPR
* Skilled in EMR documentation, with strong organizational ability
* Confident working independently in a home environment
* Compassionate, client-focused, and a strong communicator
* Experienced with G-tube, trach, or vent care
* Licensed to drive with reliable transportation
