Join a communityfocused primary care practice.

Green Mountain Family Practice is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team at Central Vermont Medical Center in Northfield, Vermont. In this role you will step into a wellestablished patient panel, practice fullspectrum family medicine in a modern, integrated clinic, and support student health at nearby Norwich University-all within a closeknit team that values collaboration, continuity, and patientcentered care.

What You'll Do

  • Provide outpatient family medicine across the lifespan

  • Manage an inherited panel of ~700 patients

  • Deliver acute, chronic, preventive, and basic procedural care

  • Support Norwich University student health (includes 1 weekend/year)

  • Optional overnight home call with stipend

  • Document in Epic

  • Collaborate with onsite physicians, APPs, and integrated services

Schedule

  • 30 hours/week, with flexibility in how hours are structured: three 8hour days plus 6 hours of admin time

  • Thursdays are required as an anchor day for team coverage and continuity

  • 6 hours/week admin time, remote optional

Qualifications

  • NP or PA with (or eligible for) VT licensure

  • Commitment to patientcentered primary care

  • Comfortable working on a small, collaborative care team

  • Preferred:2+ years of primary care experience + familiarity with Epic

  • New grads welcomewith strong family medicine rotations
