Job Summary:

Southwest Healthcare is seeking a Full-Time Regional Director of Case Management. This position will be responsible for providing leadership in the development, implementation, and oversight of a care coordination model which will support a patient-centered care delivery model at multiple facilities across the region. This position is responsible for the development and implementation of standards, systems, policies, and procedures in alignment with organizational strategic initiatives and that is focused on quality and financial outcomes. Specifically, this position leads region wide efforts to optimize care coordination across the care continuum. Duties include but not limited to:

  • Participates in Emergency Preparedness Drills. Demonstrates through practice drills and upon request understanding of his/her role in the event of an emergency or disaster.
  • Facilitates departmental and hospital educational activities and inservices in conjunction with the Education Department.
  • Responsible for development, review and revision of departmental written policies and procedures.
  • Ensures that the department has equipment, supplies necessary to provide consistent quality and service delivery.
  • Reports any unsafe situations or safety hazards immediately. Labels and removes any malfunctioning equipment from service and notifies Plant Ops.
  • Position requires travel between sites throughout the Southern California region to support team collaboration, education, and training efforts.

Qualifications

EDUCATION/TRAINING/ EXPERIENCE:

  • Bachelor's degree from an accredited College or University in related field required, BSN preferred.
  • Master's degree from an accredited College or University in Business and/or Healthcare Administration preferred.
  • Eight (8) to Ten (10) years of clinical experience as a Registered Nurse required.
  • Minimum three (3) years acute care nursing leadership experience in Case Management required.

CERTIFICATIONS/LICENSES:

  • Current license as a Registered Nurse in the State of California reuqired.
  • Current CA Drivers License required.

OTHER SKILLS AND ABILITIES:

  • Demonstrates knowledge and ensures compliance with the The Joint Commission and Title 22 standards and guidelines.
  • Demonstrates compliance with healthcare policies and procedures at all times.
  • Demonstrates strong leadership, organization, communication and interpersonal skills.
  • Demonstrates ability to relate to clinical personnel and medical staff, as well as ability to interact well with the public.
  • Must have knowledge of PC and applications
  • Develops plans of corrective actions with Corporate and Regional Care Management partners.
  • Schedules periodic site visits to provide supervision, clinical expertise, ensure communication, and to monitor the overall operation the clinical services department.
  • Conducts facility reviews based on operational needs, changes in management and commitment to regulatory compliance.
  • Provide in-service training and orientation to facility staff regarding survey process, care management, corporate compliance, performance improvement, utilization management, etc.
