Rutgers, The State University of New Jersey, is seeking a Patient Navigator for the department of OB/
GYN at the Robert Wood Johnson Medical School.
The Patient Navigator supports the improvement of access and patient outcomes through implementation of evidence-based patient navigation services for reproductive genetic counseling throughout New Jersey. The Patient Navigator serves as the liaison between referring offices, triages the requests for appointments and makes sure that patients are scheduled promptly, all pertinent records collected and patients are prepared for their visit with the genetic counselor, Maternal Fetal Medicine physician and the Antenatal Testing Unit (
ATU). The Patient Navigator provides individualized assistance for referring offices and patients to help access appropriate services. The Patient Navigator also makes sure patient samples are properly collected at designated draw sites and the
ATU, sent to various labs across the country (as needed) and that all results promptly get reported back to the patient chart for management and communication. The Patient Navigator also assists the patients and clinical personnel with submitting benefit investigations for complex genetic testing and works with insurance companies to advocate for access and coverage. The Patient Navigator serves as the primary liaison between referring practices as serves as point person to assist with providing timely reporting and communication. The Patient Navigator will serve as a key resource for developing and expanding patient access to care resources and overcoming barriers to screening and improved access to support resources.
Among the key duties of this position are the following:
- Educates patients on genetic services, care management, and available resources. Directs and implements care coordination plans in both inpatient and outpatient settings for maternal fetal medicine and genetics.
- Coordinates inpatient and outpatient care plans for Maternal Fetal Medicine (MFM) and Genetics, Obtains patient clinical information ensuring timely scheduling and followup and referrals. Works with MFM and genetics administrative team to make sure all schedules are coordinated appropriately for all clinics and offsites.
- Communicates with physicians, patients, family members or patient representatives to establish patient's plan of care, goals and expected outcomes.
- Assists genetic counselors and physicians with all coordination aspects of patient care.
- Verifies insurance eligibility and provides guidance around referral and managed care issues, and information about other prior authorizations that may be needed for testing and procedures.

Qualifications
| Minimum Education and Experience |
- Bachelor's Degree in a Health, Public Health, Social Work, or other relevant field required; plus two (2) years' experience in health or community outreach/advocacy related field.
- Equivalent experience, education and/or training may be substituted for the education requirements.
