In thisHealth and Social Services Coordinator RNrole, you will provide service coordination within an integrated care model supporting medical, behavioral health, and Long-Term services and supports (LTSS). This is a Field Based position with a Home-Based office. Work Schedule:Monday - Friday, 8am - 5pm CST If you are located in the Greater Houston area, you will have the flexibility to work remotely* as you take on some of the most meaningful challenges in healthcare. The hourly pay for this role will range from $28.94 to $51.83 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
