In thisHealth and Social Services Coordinator RNrole, you will provide service coordination within an integrated care model supporting medical, behavioral health, and Long-Term services and supports (LTSS).

This is a Field Based position with a Home-Based office.

Work Schedule:Monday - Friday, 8am - 5pm CST

If you are located in the Greater Houston area, you will have the flexibility to work remotely* as you take on some of the most meaningful challenges in healthcare.

Primary Responsibilities:

  • Assess, plan, and implement individualized, person-centered care strategies driven by the member needs and preferences and directed toward the least restrictive, most appropriate environment
  • Assess member needs and coordinate referrals to community-based and state-supported programs, including financial, psychosocial, and other supportive services
  • Oversee the member's Plan of Care across settings, serving as a consistent point of contact to support person-centered care coordination
  • Communicate health and social needs information across stakeholders to support timely access to person-centered care and services
  • Advocate for members and their families to ensure the members' needs and choices are fully represented and supported by the interdisciplinary care team in accordance with person-centered service planning principles
  • Make and receive calls to assess members' current health status, functional needs, and potential changes in condition, in support of care planning
  • Translate clinical and social information into actionable care plans

Required Qualifications:

  • Current, unrestricted RN license in the state of TX
  • 2+ years of experience working within the community health setting or health care role
  • Intermediate level of proficiency with Microsoft Office, including Word, Excel, Teams, and Outlook
  • High-speed internet at residence
  • Access to reliable transportation and valid US driver's license to meet job-related travel requirements

Preferred Qualifications:

  • Case management or service coordination certification
  • Experience in LTSS, Medicaid/Medicare, or managed care experience
  • Strong documentation skills with the ability to maintain timely, accurate, and compliant records
  • Knowledge of integrated setting principles, including federal and State requirements like the federal home and community-based settings regulations
  • Reside within commutable distance of assigned duties

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $28.94 to $51.83 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
