Overview

Illinois CancerCare is seeking to hire a Patient Navigator Coordinator to support our new benign hematology patients through their initial steps in care. This role is part of an expanded navigation model that allows the New Patient RN Navigator to focus on oncology patients while maintaining a consistent, high-quality experience across both patient populations. Both roles will operate within the overarching Navigation framework, and both will be trained through the American Cancer Society's LION program, enabling them to contribute to PIN billing eligibility.

Responsibilities

* Navigate benign hematology new patients.

* Provide non-clinical education, appointment guidance, barrier assessment, pre-visit support, referral coordination, and first visit preparation.

* Acts as a liaison and advocate for patients, families and caregivers when assisting the multidisciplinary team.

* Serves as a liaison/contact for referring physicians, external medical offices and benign hem patients in a timely, professional manner.

* Close referral loops and improve scheduling readiness.

* Support the New Patient RN Nurse Navigator in reporting patient and family challenges and assist with potential solutions, while considering the patient's values and cultural, spiritual and social needs.

* Reduce administrative load on nursing and scheduling teams.

* Participate in metric tracking (TTF, no show rate, referral completeness).

* Coordinates patient needs for appointments and schedules appropriately to expedite the plan of care.

* Appropriately documents all patient interactions and services provided within the electronic medical record.

* Participates in continuous process improvement of patient outcomes, assuring appropriate utilization of resources to increase patient, family, and caregiver satisfaction.

* Complete the ACS LION certification for Navigation competency + billing support.


Qualifications

* High school diploma or GED required; advanced education preferred

* Minimum four (4) years of experience working in a clinical health care environment is required.

* Oncology-related education, planning, management, or similar experience is required.

* Medical Assistant experience with oncology/hematology patient population preferred
