Bulfinch Temps is hiring a temp NP to cover a maternity leave for the BWH Community Care Vans. Weekends and Some evenings are required. Please note this position is temporary in nature and not benefitted. The role is 24-40 hours per week, must be available for over 20 (weekends and some evenings). Must have 3 years experience as a NP and additional language skills are a plus!

The Mass General Brigham (MGB) Community Care Van is a mobile health program designed to expand access to high-quality, culturally responsive care for communities that experience barriers to traditional healthcare settings. The interdisciplinary team delivers care directly in community locations, meeting patients where they are. The Nurse Practitioner is a core clinical member of the mobile team that provides patient-centered services including cardiometabolic-focused visits, colorectal cancer screening, substance use disorder screening and care, vaccinations, and screening for social determinants of health with connection to resources.

The NP provides direct clinical care during approximately 5-6 mobile clinic sessions per week at multiple community sites, supporting clinic set-up and flow, coordinating with on-site staff, and ensuring timely, high-quality patient service. As clinics occur in community settings without immediate on-site clinical back-up, the NP must practice with a high degree of independence and clinical judgment, with demonstrated ability to anticipate, prevent, and manage complex presentations and urgent/emergent situations for patients across the lifespan.

Clinics are often held in the evenings and on weekends. Locations vary but are primarily located in Boston neighborhoods with occasional clinics in other communities such as Chelsea and Lynn.

The NP may also be asked to assist with follow-up calls to patients to support blood pressure management.

Functions independently and uses EPIC to provide care for all patients seen by the mobile team. Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or emergency patient care problems. Maintains and practices in accord with practitioner guidelines per state and hospital regulations. Works in other mobile clinical roles if needed due to a staffing shortage or high patient volume. Provides primary care services in affiliated settings as needed.

  • Performs a physical exam, obtains medical history and records findings. Orders appropriate laboratory and diagnostic tests. Interprets and integrates data to determine diagnosis and therapeutic plan.
  • Administers medications and injections in compliance with state law and clinic guidelines.
  • Actively supports patient safety program; adheres to applicable patient safety standards.
  • Instructs patient/family regarding medications and treatments. Educates patients regarding health and illness prevention. Recommends community resources as needed to meet patient and family needs.
  • Maintains and reviews patient electronic medical record and other pertinent information. Prepares written documentation as required in the patients medical record following standard clinical practice.
  • Collaborates with physicians in managing acute and long-term medical needs of patients.
  • Provides clinical results and consultation to patients over the phone in follow-up to care provided during the mobile clinic.
  • Treats all patients courteously and with dignity and respect at all times.
  • Maintains patient confidentiality

Serves as a resource to staff in area(s) of expertise. Collaborates with Mobile Operations Manager to ensure adequate clinical resources for each mobile clinic. Performs other duties as assigned.

Current Massachusetts Licensure as an NP required, Family Nurse Practitioner preferred

DEA-X waiver preferred, must be willing to obtain
