Job Description

As a Care Coordinator, you will be a key part of the Care Connection team and one of the primary points of contact for patients beginning and continuing their primary care journey. In this role, you will help create a welcoming, seamless, and supportive healthcare experience by guiding patients through appointments, answering questions, helping remove barriers to care, and partnering with both clinical and non clinical teams to ensure every patient feels supported every step of the way. What You'll Be Doing

  • Serve as a central point of contact for coordination and oversight of primary care scheduling duties and responsibilities.

  • Use proper protocols to facilitate access to healthcare by reaching out to non established primary care patients, answering incoming calls, identifying patient needs, scheduling physician appointments, verifying insurance information, communicating policies and procedures, and placing reminder calls regarding appointments, prescriptions, and follow up care.

  • Act as a non clinical liaison and concierge for referred patients by responding to requests for information on services available within the organization and providing accurate and timely information.

  • Accurately track and manage data collection to demonstrate program outcomes and support operational goals.

  • Support the Care Connection Nurse Navigator and Care Connection team members to ensure continuity of care throughout the patient experience.

  • Build positive and professional relationships with patients, providers, and hospital staff with a strong focus on customer service and quality care.

  • Provide outreach and support to patients, including coordinating and promoting access to myUHealth, while collaborating with clinical and non clinical personnel to identify and eliminate barriers to care.

  • Promote quality improvement, staff and patient safety, and cultural diversity through daily operations and personal performance.

  • Provide service in a manner appropriate for the patient's age and demonstrate the knowledge and skills necessary to meet physical, psychosocial, educational, and safety needs.

  • Maintain a courteous, professional, and helpful demeanor with patients, visitors, employees, medical staff members, and all individuals encountered while representing University Health.

  • Maintain current knowledge of applicable statutes, regulations, guidelines, and standards necessary to perform job duties in accordance with the Corporate Compliance Plan, Code of Conduct, and Compliance Policies and Procedures. Participate in required compliance activities and training.

  • Adapt to change, accept new assignments, and continuously seek ways to improve processes and patient experiences with enthusiasm and professionalism.

What You Bring (Requirements)

  • High School diploma or equivalent required.

  • At least one year of experience in a clerical, administrative, healthcare, or customer service setting required.

  • Must possess computer keyboarding skills of at least 30 words per minute and basic knowledge of Microsoft computer programs.

  • Excellent interpersonal and communication skills required.

  • Must be able to read and speak the English language.

  • Strong ability to provide exceptional customer service.
