DESCRIPTION OF WORK

This Complex Case Coordinator for Dual Diagnosis and Cross-System services is a key a central point of contact for multiple social service systems including Mental Health, Developmental Services, Physical Health, and more. You will help our teams and community partners work together to proactively address the needs of people we serve. Duties in this role include:
- Cross-System Coordination: Cofacilitate and document support, planning, and staffing efforts to serve individuals who are involved in multiple program areas; collect and analyze data to support future evidence-based advocacy and program expansion
- Provide Technical Assistance: Help internal and external partners improve their technical skills or solve specific problems related to complex case coordination
- Complex Case Connections: Collaborate with providers and support coordinators to gather existing plans, evaluations, and any other information available; provide referrals to appropriate education and assistance
- Build Community Relationships: Answer phone calls for new complex cases; participate in ongoing communication with individuals and families to identify needs and provide connections to services
- Further Case Progress: Facilitate admission, discharge, and transition meetings for individuals moving in and out of psychiatric inpatient units; collaborate to provide assistance and education support to individuals moving from youth to adult services
- Transition Follow-Up: Coordinate cases and follow-up for individuals moving in or out of the county as well as individuals transitioning from higher levels of care back into the community
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. Occasional need to work or attend meetings after hours or on weekends. Compensation provided.
- Telework: You may have the opportunity to work from home (telework) part-time, up to three days of telework per week, after successful completion of a six month probationary period and director approval. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Greensburg. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Two years of professional experience in the field of intellectual disabilities, and a bachelor's degree; or
- An equivalent combination of experience and training..
Other Requirements:

- You must be able to perform essential job functions.
Legal Requirements:
- A conditional offer of employment will require a drug screening.
- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law.
