About the RoleThe Senior Community Care (SCC) program delivers coordinated, patient-centered care to health plan members. As an Advanced Practice Clinician (NP or PA), you will provide comprehensive clinical services to some of our most vulnerable patients, helping improve outcomes and quality of life. Key Responsibilities
  • Conduct comprehensive patient assessments, including:
    • Medical history reviews
    • Medication reconciliation
    • Physical examinations and vital sign assessments
    • Evidence-based screenings and point-of-care testing, as appropriate
  • Support members during short-term skilled nursing and rehabilitation stays
  • Ensure timely and accurate clinical documentation, including ICD-10 diagnosis coding
  • Collaborate closely with physicians, nurses, care coordinators, and other healthcare professionals
  • Facilitate advance care planning and discussions regarding treatment goals
  • Educate and counsel patients and caregivers on health conditions and care plans
Required Qualifications For Nurse Practitioners
  • Current ANCC or AANP certification in:
    • Family
    • Adult
    • Geriatric
    • Adult-Gerontology Primary Care
For Physician Assistants
  • Current NCCPA certification and applicable state licensure
Additional Requirements
  • Active, unrestricted state license (or ability to obtain before start date)
  • Active, unrestricted driver's license
  • Reliable transportation
  • Current DEA license/prescriptive authority, or ability to obtain after hire, subject to state requirements
  • Ability to establish a collaborative practice agreement if required by state law
  • Ability to perform job duties across various patient care environments
Preferred Qualifications
  • 1+ years of experience as an Advanced Practice Clinician, preferably in long-term care
  • Experience in geriatrics, senior living, home care, or long-term care settings
  • Ability to care for patients with complex medical, behavioral, social, and functional needs
  • Knowledge of chronic disease management and geriatric care
  • Experience leading advanced illness and end-of-life care conversations
  • Proficiency with electronic medical records and healthcare technology
Compensation & BenefitsCompensation for this specialty generally ranges from $53 to $79 per hour, based on factors including experience, education, location, and market conditions. In addition to competitive pay, eligible employees may receive:
