We are seeking a qualified and caring person to work as an RN Healthcare Coordinator to provide healthcare coordination and direct intervention, as needed, for designated individuals.

Full-time position Monday - Friday

RN MUST be familiar and experienced in Division of Developmental Disability Services (DDDS)

Responsibilities:

  • Provide training for unlicensed staff to meet the healthcare needs of individuals served.
  • Contribute to the clinical oversight of program LPN, as applicable.
  • Provide healthcare and wellness coordination for individuals in accordance with all state and federal regulations.
  • Function as liaison and advocate for individuals' healthcare with all members of individual medical and interdisciplinary teams. Advocate for their wellbeing and inclusion.
  • Train unlicensed staff on medication administration, delegated tasks, treatment protocols, and monitoring.
  • Become a LLAM certified training in the state of DE
  • Implement and train employees on new and revised health related policies and procedures in a timely manner as directed.
  • Oversee pharmacy and medication needs This may include but is not limited to reviews, obtaining medications, and prior authorizations.
  • Order medical supplies from established formularies as applicable for your program.
  • Maintain health-related records, complete documentation and assessments per company policy, federal, and state regulations, as applicable.
  • Support individuals in obtaining health services appropriate for their preventative, acute, and/or chronic illness needs.
  • Oversee that the health status and physical conditions of individuals are assessed (hands-on), monitored, reported, and responded to in a timely and appropriate manner.
  • Respond to emergency medical situations in a timely manner, as appropriate.
  • Participate in and attend assigned medical individuals' appointments (as directed), individual specific, nurse, departmental, and other meetings as required.

Qualifications:

  • RN with current DE license.
  • Experience in IDD/Mental Health nursing.
  • Experience with the Division of Developmental Disabilities Services (DDDS) in Dover
  • Experience working with individuals with intellectual, developmental disabilities and/or mental illness.
  • Valid Driver's License, with no driving limitations.
