As a Primary Care Physician with 'UCSD at Home'- Population Health Services, you will have the unique opportunity to provide high-quality, personalized medical care to senior patients through both in-home visits and telemedicine platforms.

KEY RESPONSIBILITIES
  • Telemedicine Consultations: Conduct virtual patient assessments, diagnose eligible medical conditions, and provide treatment plans and follow-up care through telemedicine platforms

  • Comprehensive Assessments: Conduct thorough in-home medical assessments of senior patients via either in person or telemedicine, including reviewing medical history, physical examinations, and diagnostic tests as needed

  • Preventive Care: Educate and encourage seniors to engage in preventive care measures, vaccinations, and health-promoting lifestyle choices through both in-home and telemedicine consultations.

  • Chronic Disease Management: Manage and monitor chronic conditions in senior patients, optimizing health outcomes, oversee medication management, compassionate palliative care, patient and family education through in-home visits.

  • Collaboration: Collaborate effectively with interdisciplinary 'UCSD at Home'- Population Health Services healthcare teams, including Physicians, APP, nurses, social workers, and specialists, to ensure holistic and timely care.

  • Documentation: Maintain accurate and up-to-date electronic health records (EHR) of patient encounters, treatment plans, and progress notes for both in-home and telemedicine visits.

MINIMUM QUALIFICATIONS
  • Medical degree (MD or DO) from an accredited institution.

  • Active and unrestricted medical license in California.

  • Board certification in Family Medicine, Internal Medicine, or Geriatrics is preferred.

  • Proven experience in primary care, particularly in geriatric medicine.

  • Strong interpersonal and communication skills, with the ability to build rapport with senior patients and their families.

  • Compassion, empathy, and cultural sensitivity when working with diverse senior populations.

  • Strong organizational and time management abilities.

  • Familiarity with telemedicine platforms and technology.

  • A commitment to providing patient-centered, in-home and telemedicine-based medical.

Salary is based on training, experience and local labor market. The posted annual salary range is the starting annual compensation only. The amount does not include productivity or incentive plans.
