Job Summary Role and responsibilities of oncology outpatient IDS pharmacy patient care services. Oncology experience and/or investigational drug service is preferred. Hours are Monday - Friday 7:30pm - 5:30pm. Experience and Education
- Bachelor of Science degree or PharmD from ACPE accredited school of pharmacy and 1 year with experience in a hospital setting required for a hospital position, 1 year retail experience for retail position preferred.
- Eligible for licensure by Texas State Board of Pharmacy (TSBP) is required and Texas license with TSBP within 30 days of date of hire.
- Current Basic Life Support (BLS) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC).
- Sterile compounding (IV) certification accredited by ACPE.
Job Duties
- Performs job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives, environmental and infection control standards, as well as state and federal laws and regulations.
- Performs computer order entry in accurate, thorough and timely manner.
- Reviews and updates medication profiles and is cognizant of patient charges.
- Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety.
- Seeks clarification if necessary and documents interventions.
- Works and problem solves with minimal supervision.
- Takes charge when necessary and is looked to for direction.
- Makes right decisions and uses good judgement. Is cross trained and multi-functional maintaining competency in multiple areas.
- Enforces formulary system and follows departmental policy and procedure when acquisition of NF product is judged necessary.
- Participates in cost effective use and selection of pharmaceuticals.
- Communicates drug information to physicians, nurses, etc. in courteous and professional manner.
- Assists in patient and family education and counseling regarding drug therapy and side effects.
- Documents clinical activities, demonstrating cost savings.
- Conveys key information required for follow-up by fellow pharmacists.
- Manages technicians and other supportive personnel.
- Delegates appropriate and reasonable tasks and validates accuracy of floor stock replacements, IV batches, cart fills and other drug doses dispensed from pharmacy.
- Audits and signs out all controlled substances in accordance with state and federal guidelines and organizational policies. Investigates and corrects discrepancies and unusual usage patterns and trends.
- Enhances personal growth and development.
- Participates in educational programs, in-services and meetings.
- Participates in new employee orientation and training.
- Takes on additional projects as needs develop.
- Participates in clinical program.
- Supports P&T initiatives.
- Monitors target drugs for protocol discontinue, substitution, IV to PO, etc.
- Collects applicable data and measures and communicates opportunities for improvement.
- Performs therapeutic monitoring of specified drug therapies and recommends dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations.
- Performs other duties as assigned.
