DESCRIPTION

Population Health Registries and data models are pivotal in ensuring patients receive the right care at the right time. More than just co-horting tools, they aggregate payer data and clinical outcomes, delivering critical insights into value-based care. These registries act as the engine driving healthcare by guiding decision-making and optimizing care pathways. However, the interconnected processes and complex rules that underpin these systems require both precision and advanced expertise to manage effectively.

The role of the enterprise population health architect is essential in designing, building, testing, and implementing these registries and data models within clinical application systems. This position requires a deep understanding of Epic Healthy Planet, data architecture, accurate groupers, rule-based decision support tools, health maintenance, bulk outreach, communication strategies, and the reporting workbench. The architect collaborates with clinicians to gather detailed specifications on clinical processes, information flow, and risk analysis, ensuring that the software adapts to meet the needs of healthcare providers while maintaining clinical practice standards.

As a technical leader in population health, the architect works on medium to large projects of broad scope and complexity, often recognized as an expert in clinical applications. This role involves performing quality assurance testing of complex systems and providing essential support through troubleshooting, customer service, and system maintenance. Additionally, the architect is responsible for creating and maintaining comprehensive documentation, including design and deployment processes, workflows, and outcomes.

Through a combination of advanced technical knowledge and clinical insight, this role ensures that population health registries not only improve patient outcomes but also streamline workflows and enhance the efficiency of healthcare delivery. The ability to envision cross-enterprise solutions that can be repurposed into multiple workflows ensures the systems remain flexible and scalable across various healthcare settings.

This position offers flexible work arrangements, allowing for either a local hybrid schedule within commuting distance of San Diego or 100% remote work from anywhere within the United States. Candidates opting for remote work must adhere to an 8-hour shift between 7:00 AM and 5:00 PM Pacific Time, Monday through Friday. Additionally, after three months of joining the team, the candidate will participate in an on-call rotation for two weeks at a time, ensuring continuous support and responsiveness to system needs.

MINIMUM QUALIFICATIONS
  • A minimum of seven (7) years of related experience or education/training in clinical application systems, Population Health, or healthcare workflow solutions is required. Alternatively, candidates with a Bachelor's degree in a related field and at least three (3) years of experience in these areas will also be considered.

  • EPIC Certification in EpicCare Healthy Planet, and/or EpicCare Ambulatory, and/or EpicCare ClinDoc or obtain certification(s) within four (4) months of hire.

  • Must maintain current certification for primary EMR module(s) as a condition of employment.

  • Experience and proven success in information systems implementation, development and support within a large-scale healthcare organization.

  • Proven interpersonal skills and ability to manage multiple projects independently, setting priorities to meet deadlines.

  • Excellent verbal, written communication skills, presentation and report writing.

PREFERRED QUALIFICATIONS
  • Experience in a large, complex healthcare environment.

  • Extensive experience and working knowledge of all MS office products. Knowledge and experience of database management technologies.

  • Strong project management skills.

SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.

Pay Transparency Act

Annual Full Pay Range: $94,400 - $176,800 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $45.21 - $84.67

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
