The Solid Organ Transplant Clinical Pharmacist provides specialized patient-centered clinical services to the inpatient solid organ transplant population in addition to the outpatient transplant clinic/infusion center. This position must: provide pharmaceutical care to patients to achieve optimal therapeutic outcomes; promote cost-effective drug therapy; promote cooperative work environment by utilizing communication skills, interpersonal relationships and team building; assist with verification and dispensing of medication orders; provide assistance in educating other health care professionals in regards to pharmacotherapy; participate in multidisciplinary and cross-service drug therapy management plans; assist in developing and implementing treatment protocols in order to improve patient care. This person must possess the ability to work independently, demonstrate a positive attitude, have a strong commitment to teamwork and customer service as well as strive to consistently improve patient care by working as a cohesive transplant pharmacy team to achieve goals.
MINIMUM REQUIREMENTS Education: Bachelor's in Pharmacy or Pharm. D Degree required. Experience: PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas. Licensure: Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider. BLS certification required PHYSICAL DEMANDS Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease.
