Overview

LifeLong Medical Care is looking for a Psychiatric Nurse Practitioner or Psychiatric Physician Assistant committed to providing high-quality outpatient primary care at our Over 60 Health Center (OSHC). The ideal candidate must be dedicated to building a collaborative team model of care experience working with the under-served. Case management and behavioral health services are fully integrated into our care to address various medical and psychosocial factors impacting the patient's overall health. The Health Center utilizes a multidisciplinary team approach with primary care providers, RNs, Social Workers, Psychiatry, Podiatry, etc.

As a PMHNP/PA at our Health Center, you'll have the opportunity to work alongside a multidisciplinary team of healthcare professionals to provide comprehensive care to our patients. Our Health Center utilizes a collaborative team model of care, integrating case management and behavioral health services to address the various medical and psychosocial factors impacting our patients' overall health.

Patients are referred for psychiatric care from all Health Center/Program locations throughout Alameda and Contra Costa counties

This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA.

Benefits:

Compensation: $142,000-$163,000/year and pro-rated depending on years since licensed, sign-on bonus, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 16). Acceptance rates may vary year to year based on funding levels and the number of applicants.

Responsibilities

    Conducts psychiatric assessments and administers treatment of all referred primary care patients.
  • Offers diagnostic and treatment consultation to primary care providers and other clinicians.
  • Develops and assists in implementing psychiatric treatment plans with the patient's primary care provider and other clinical providers or staff.
  • Participates in case conferences, staff meetings, and phone conversations to coordinate clinic patient services. All services will be provided in collaboration with other behavioral health clinicians, service coordinators, case managers, social workers, and primary care clinicians.
  • Provides ongoing training and guidance to clinic team members around psychiatric issues relevant to primary care.
  • Actively participates in regular meetings of all of the LifeLong psychiatrists.
  • Helps conduct functional assessments of patients and makes appropriate referrals when indicated.
  • Documents mental health disabilities in partnership with other clinic staff and help patients with disability program applications and appeals when necessary.
  • Provides integrated mental health and substance abuse treatment services and patient planning.
  • Helps develop and conduct staff training regarding psychiatric care when appropriate.
  • Participates in ongoing program development and improvement activities.
  • Completes necessary documentation for clinical records and billing.
  • Performs other duties as assigned by supervisor.
  • Participates in Utilization Review/Quality Assurance/Continuous Quality Improvement activities based on organization, site, and specialty goals.
  • Supports educational efforts which promote mental health in the community.
  • Participates in professional development activities.
  • Participates in LifeLong Clinician Association activities and agrees to abide by their Guidelines and Code of Conduct.


Qualifications

  • Ability to communicate effectively and work compassionately with individuals from diverse backgrounds.
  • Commitment to a community health philosophy of prevention and primary care.
  • Willingness to work in non-traditional settings.

Education & Experience

  • Must possess a current CA Nurse Practitioner license and Board Certification as a PMHNP or PA-C, CAQ-Psych license.
  • DEA and furnishing license with the ability to obtain within one year.
  • Training and experience in public sector work, community mental health, and working with dually diagnosed clients.
  • Strong computer skills required, including EHR Epic.
  • Fluency in Spanish preferred
