The Case Assessment Specialist is responsible for evaluating and coordinating potential patients through the continuum of care into rehab and proactively plans and functions as the liaison between patient/family and/or referral sources with the rehab hospital. • Licensed clinician in the State of Oklahoma
• 2 years of experience health care setting; rehabilitation facility preferred
• Valid Oklahoma driver's license and automobile insurance verification
• Must be able to communicate effectively in English (verbal/written
