HonorHealth is looking for a RN Navigator to support the Ortho Institute. This team is made up of Case Managers, Community Ortho Surgery Schedulers, PeriOp RNs, PTs and more. This is a new department and a new role with lots of opportunity to help determine work-flow through input and feedback. With this role we are also able to bring back the Total Joint class across the Network. This role will work with high risk total joint patients before surgery to confirm medical clearance and office visits, and follow up appointments with Primary Care Providers. You will also facilitate the total Joint class, responsible for RN rounding while patients are hospitalized while working with the Quality Department and tracking data to show the success rate with the program. The ideal candidate must meet the minimum requirements of Bachelor's degree in Nursing (BSN), five (5) years clinical experience in a hospital or ASC, three (3) years clinical experience working with surgical patients along with a current RN license with the State of Arizona or a Compact State license, and a current Basic Life Support (BLS) card. To ensure success in the role the candidate must be able to work independently and autonomously, be a "go-getter" and takes the initiative along with having a heart for Ortho, positive energy and a good attitude. Responsibilities Job Summary The Orthopedic Nurse Navigator is a Registered Nurse with orthopedic and Total Joint-specific clinical skills and comprehensive knowledge of Total joint arthroplasty, pre-operative and post operative treatment modalities, and preoperative medical optimization. Responsible for patient coordination, episode of care coordination, risk assessment, post discharge follows up and organization with priority focus in the high-risk patient to decrease preventable re-admissions and postoperative complications and improve quality of care and patient satisfaction. The Orthopedic Nurse Navigator will demonstrate commitment to quality patient care, implement creative and innovative ways to meet diverse, complex patient needs and ensure best practices and standard of care for total joint patients are followed. Serves as clinical advocate and educator for patients. The Orthopedic Nurse Navigator will build and maintain relationships with orthopedic surgeons, APP's, office surgery schedulers and staff to provide reliable consistent communication regarding high-risk total joint patients. The position is associated with a significant level of autonomy; therefore, the individual must be highly motivated and able to work independently. Demonstrates skill as a clinician and educator.
